Better health status among women, men, children and adolescents with the help of ‘Life for Child Programme’ through KISES Community Health Centre

Project area: Machilipatnam Rural, Pedapatnam Village, 30 villages

ABOUT THE ORGANISATION:

Kiranmayi Socio Educational Society (KISES) is a non-governmental organisation based in Khammam. It was established and registered in 1999 with the aim to contribute to the physical and mental well-being of the marginalized and vulnerable sections of society, particularly women and children. Kiranmayi Socio Educational Society initiated its activities by conducting various Health Camps, Health Melas and Free Medical Services including free distribution of medicines in various villages of Machilipatnam rural with the help of an expert team of doctors. Subsequently, KISES has implemented eye camps, dental camps, child nutrition, immunisation, birth registrations, reproductive child health services and hygiene practices in various villages of Machilipatnam rural area. 

HEALTH CENTRE:

The Health Centre is located at Pedapatnam Village, Machilipatnam mandal, Krishna District, Andhra Pradesh, India. The centre is about 20km from Machilipatnam town. The centre was constructed to cover the nearest 30 villages, covering a population of 27,000. There is no Primary Health Centre within the area for basic health services.  

OBJECTIVES OF THE ORGANISATION:

Major areas are following:

a.   Livelihoods: To enhance social and economic capacity among the rural people.

b.   Health: To provide basic health services for men, women, and children.

c.   Women empowerment: To strengthen and sustain women through micro enterprises and income-generating activities.

d.   Agriculture: To sustain the rural people through agricultural development and innovative methods.

e.   Child Development: To gain better health & education status in children in distress.

f.   Environment: To create a better, healthier environment in rural and urban areas.

g.   Disaster management: To rehabilitate people who are in distress due to disasters.

PROJECT GOAL:

Better health status among men, women, children and adolescents with the help of the KISES Community Health Centre.

PROJECT OBJECTIVES:

  • To provide health education on diabetes and new treatments through group counselling, experiences and skills sharing to senior children, youth and parents.
  • To increase awareness on insulin vials, syringes/pens, blood glucose meter and strips and other medical supplies. 
  • To reduce mortality through training and awareness campaigns.
  • To provide basic health services to the children and youth who are in need.
  • To provide basic health care services in a regular and a systematic manner.
  • To refer cases to the concerned hospitals.
  • Counselling by the doctor and KISES staff and volunteers on the adoption of good health practices.
  • To provide health education through materials like posters and pamphlets.

 NEED OF THE PROJECT: 

Nearly 30 villages are situated around Pedapatnam village, covering 27,000 people. As there is no primary health centre within a 20km radius of the operational villages the people are suffering from lack of health services. Most of the villagers are not aware about diabetes in children and adolescents, insulin utilisation, syringes utilisation, HbA1 testing and blood glucose testing. Diabetes education is needed by the people of these villages to sustain and extend child life. So the Life for a Child Programme is very much needed in this area.  

INFORMATION ABOUT THE PLAN OF ACTION FOR THE COMING PERIOD

1. METHODOLOGY:

A. Location:

The Health Centre will be located in the middle of the villages where everybody can access the medical services easily.

B. Schedule:

The doctor will be available in the Health Centre 5 days a week (Mon-Fri). The timings of the Health Centre will be 9am-1pm. The schedule will be reviewed and adjusted as required by the villagers.

C. Target Area:

30 villages circling the Health Centre, Pedapatnam, Machilipatnam rural, Krishna District, Andhra Pradesh, India.

D. Target Group:

The entire villages, with special focus on women and children of the target area.

E.  Project Period:

Initially one year and extendable

F.  Physical features of the Health Centre:

The Outpatient Department will have the following features:

2. The Health Centre will have the basic facilities such as:

  • Glucose meters and test strips
  • Syringes
  • Insulin storage facility
  • Hb A1 Testing kits
  • Diabetes education material
  • Table and Chairs
  • Beds
  • Examination bed and two stools
  • Rubber sheet, bed sheet, towels etc.
  • Almirah ( to store medicines, first aid, papers, documents and other consumables)
  • Fans
  • Refrigerator
  • Cooler
  • Required lights etc
  • Wheel Chair

3. The basic equipment which is required:

  • Thermometer
  • Weighing scale
  • Stitching Tray
  • Sphygmomanometer
  • Ostoscope
  • Ishara charts (Eye charts)
  • Steel drum for keeping sterile gauge

  • Height measuring apparatus
  • Step table for stretcher

4. The human resources to provide the primary health care will constitute:

  • Medical Doctor
  • Nurse 
  • Medicine dispenser cum Health Centre
  • KISES staff for occasional monitoring

OUTCOME INDICATORS:

  • The total population of the villages is 30,000. In a period of one year, at least 80% of the total population will benefit from the services of the Health Centre. Benefits include regular check-ups, counselling and education on diabetes provision of medication.
  • Sincere attempts will be made in significantly reducing the number of referrals for general ailments.
  • Only serious patients who cannot be treated in the Health Centre will be referred to hospitals and clinics.
  • At least 80% of the people from 30 villages have received treatment of diabetes at the end of one year.
  • At least 20% of the patients have been referred to concerned hospitals and received treatment.
  • Increased awareness among 70% of the target group on general health, personal health and hygiene and diabetes at the end of one year.

MONITORING:

The overall supervision of the project will be the responsibility of KISES staff, who will monitor the Health Centre once a week. The number of patients will be checked and the follow-up will be recorded on a monthly basis. A precise report will be prepared every month to give an understanding of the performance of the Health Centre, as well as the increased health status of the villagers.

SUSTAINABILITY PLAN:

The goal of the project is better health status of men, women and children with the help of regular Health Centre services. Bringing good health to the community is not a one-time activity, but a continuous process which has to be sustainable. Health Centre services will be provided to the people of 30 villages initially for the period of one year with the help of KISES and support from a donor organization. The donor organization will be primarily supporting for the equipment and facilities required in the Health Centre, the salary for the doctor and the medicine dispenser, medicines, stationary and the conveyance to the villages.

Taking sustainability of the Health Centre (after initial one year) into consideration, KISES has taken the decision of charging a small amount of fees from every patient. The fees will be ranging from Rs.10 to Rs.20. The fees collected from the Health Centre, Pedapatnam will be saved and this money will be utilized in the consecutive year. KISES will take an initiative to form a health committee in the village. The health committee will include the village level health workers and some of the active panchayat and other members of the village. The health committee will take an active role in the Health Centre management.

As calculated, the money generated from the community during the period of one year will be enough for purchasing of medicines, first aid, other consumables, maintenance of the Health Centre and purchase of stationary. Salary for the doctor and the medicine dispenser is required and their conveyance. During the time period of two years the Health Committee will be stabilized and will be able to mobilize some funds from the community as well as outside sources. From the third year onwards the Health Centre in Pedapatnam will be self-functioning and self-sustaining.
